Urinary incontinence evaluation helps identify the type and cause of urine leakage and determine the most appropriate treatment. Depending on the patient’s symptoms and condition, evaluation may include a review of medical history, pelvic examination, urine testing, cough stress testing, and assessment of bladder emptying.
Urinary incontinence can present in different ways. Identifying the type of leakage is an important step in determining the most appropriate treatment.

| Stress Urinary Incontinence | Urine leakage occurs when pressure inside the abdomen increases, such as during coughing, sneezing, laughing, running, exercising, jumping, or lifting heavy objects. This is often associated with weakness or reduced support around the urethra and pelvic floor. Because the pelvic floor helps support the bladder and urethra, its strength and function are an important part of the evaluation. |
| Urgency Urinary Incontinence | A sudden, difficult-to-control urge to urinate occurs, followed by leakage before reaching the bathroom. It may also be accompanied by frequent urination, strong urinary urgency, nocturia, or other overactive bladder symptoms. |
| Mixed Urinary Incontinence | Some women experience both stress and urgency urinary incontinence. In these cases, it is important to determine which symptoms are more prominent, since each component may require a different treatment approach. |
Treatment depends on the type and cause of urinary leakage. Stress, urgency, and mixed urinary incontinence may each require different treatment approaches.
This is especially important when surgery is being considered. Urinary incontinence surgery mainly treats stress urinary incontinence and may not improve urgency or overactive bladder symptoms.
During the consultation, the doctor reviews when and how urine leakage occurs, whether it is accompanied by urgency, frequency, nocturia, or difficulty emptying the bladder, and how much the symptoms affect daily life. Childbirth history, menopause, previous pelvic or urinary incontinence surgery, medical conditions, and medications that may influence bladder function are also reviewed.
A pelvic examination may be performed to assess pelvic floor muscle strength, urethral and vaginal support, pelvic organ prolapse, and menopausal tissue changes. Depending on the patient’s symptoms and condition, evaluation may also include urinalysis, a cough stress test, measurement of post-void residual urine, or a bladder diary.
When symptoms are mixed, the type of incontinence is unclear, or more detailed information is needed before treatment or surgery, urodynamic testing may be recommended. This test evaluates how the bladder stores and releases urine and may provide information about bladder sensation and capacity, involuntary bladder contractions, stress-related leakage, urinary flow, and bladder emptying.
Treatment is selected based on the type and severity of urinary incontinence, pelvic floor strength and support, bladder function, associated urgency or overactive bladder symptoms, and individual treatment goals.
Depending on the findings, treatment may include lifestyle modification, bladder training, pelvic floor exercises or Emsella pelvic floor therapy, medication, or urinary incontinence surgery for appropriate stress urinary incontinence.
Treatment is individualized for each patient. The goal of evaluation is to identify the main cause of leakage and determine whether treatment should focus on bladder function, pelvic floor support, or both.
An evaluation may be helpful if urine leaks with coughing, sneezing, laughing, or exercise, if you experience a sudden strong urge and cannot reach the bathroom in time, or if leakage is becoming more frequent.
You may also consider an evaluation if you have started using pads, avoid certain activities because of leakage, or urinary symptoms interfere with exercise, work, travel, sleep, sexual activity, or daily life.
