In reproductive-aged patients, pregnancy should also be considered when menstruation is delayed or absent.
Evaluation begins with a review of the menstrual cycle pattern, bleeding amount, pain and associated symptoms, recent weight or lifestyle changes, medication and contraceptive use, and relevant medical history.
Depending on the symptoms, various tests may be performed, including pregnancy testing, pelvic examination, pelvic ultrasound, and blood tests. No single test can identify every cause of a menstrual disorder, so testing is selected according to the bleeding pattern, age, symptoms, examination findings, and individual risk factors.
Treatment depends on the type of menstrual disorder, its underlying cause, symptom severity, and pregnancy plans.
Patients who go for long periods without menstruation should be evaluated rather than waiting indefinitely. When ovulation remains irregular for an extended period, the endometrium may become excessively thickened.
Treatment is selected according to bleeding severity, ultrasound findings, anemia, medical history, and pregnancy plans. Heavy menstrual bleeding that affects daily life should be evaluated and does not need to be accepted as normal.
Severe or progressively worsening menstrual pain may be associated with endometriosis, adenomyosis, fibroids, ovarian conditions, or pelvic infection and may require additional evaluation.
Treatment depends on the cause. Evaluation may consider hormonal changes, ovulation-related bleeding, contraception, cervical or endometrial polyps, infection, pregnancy-related conditions, or abnormalities of the uterus or cervix.
Treatment may include medication, treatment of infection, adjustment of hormonal contraception, or removal of a polyp or other structural lesion when indicated.
Treatment may include regular exercise, adequate sleep, dietary adjustments, symptom-based medication, or hormonal treatment. Medication for mood-related symptoms or evaluation for premenstrual dysphoric disorder may be considered when symptoms are severe.
Tracking symptoms across several cycles can help determine whether they consistently occur before menstruation and improve afterward.
Similar menstrual symptoms can result from very different conditions. For example, irregular bleeding may be related to temporary hormonal changes, PCOS, pregnancy, thyroid disease, a polyp, fibroids, or perimenopause.
