Polycystic ovary syndrome (PCOS) is a hormonal and metabolic condition that can affect menstrual cycles, ovulation, skin and hair changes, weight, and fertility. Symptoms vary widely and may improve or become more noticeable over time. Treatment should therefore be tailored to the concerns, health risks, and pregnancy plans of each patient.
Polycystic Ovary Syndrome(PCOS) is a condition involving irregular ovulation, increased androgen activity, and metabolic changes such as insulin resistance. Despite its name, PCOS does not necessarily mean that true ovarian cysts are present.
PCOS may be diagnosed when other possible causes have been excluded and at least two of the following are identified:
An ultrasound finding alone is not enough to diagnose PCOS, and some patients with PCOS may not have a typical polycystic ovarian appearance.
PCOS may present differently from one patient to another. Possible symptoms include:
Some patients have a normal body weight and mild symptoms, while others experience significant menstrual, reproductive, or metabolic concerns.
The exact cause of PCOS is not fully understood. It is thought to involve a combination of genetic, hormonal, metabolic, and environmental factors.
Insulin resistance is common in PCOS. Higher insulin levels may increase androgen production and interfere with regular ovulation. Excess body weight can worsen insulin resistance and hormonal imbalance, although PCOS can also occur in patients who are not overweight.
Family history, weight changes, sleep problems, lifestyle factors, and other metabolic conditions may also affect how PCOS develops and presents.
Evaluation begins with a review of menstrual patterns, weight changes, skin and hair symptoms, medication use, family history, and pregnancy plans.
Depending on the symptoms, evaluation may include:
PCOS cannot be diagnosed by a single blood test or ultrasound finding. Other causes of irregular menstruation or increased androgen activity should be considered before confirming the diagnosis.
PCOS treatment is individualized according to the patient’s symptoms, weight and metabolic health, pregnancy plans, and long-term health risks. Treatment may change over time as these priorities change.
These treatments may help regulate bleeding, reduce androgen-related symptoms, and protect the endometrium when menstruation is infrequent.
Patients who go for long periods without menstruation should be evaluated rather than waiting indefinitely, as prolonged irregular ovulation may cause excessive thickening of the endometrium.
Hormonal treatment may help improve acne and excessive facial or body hair.
Improvement may take several months, and treatment should be selected according to pregnancy plans and possible medication risks.
Weight management is an important part of PCOS care for patients with overweight, obesity, insulin resistance, or related metabolic concerns.
For some patients, GLP-1 receptor agonist–based weight-management treatment may be considered together with lifestyle management. These medications are prescribed according to overall health, body weight, possible side effects, pregnancy plans, and the need for long-term treatment. Improving weight and insulin resistance may support metabolic health and may also help improve menstrual regularity or ovulation in some patients.
Explore GLP-1 Weight ManagementWhen pregnancy is desired, evaluation may include menstrual and ovulation assessment, pelvic ultrasound, hormone testing, and additional fertility testing when indicated.
Treatment is selected according to age, menstrual pattern, ovarian reserve, partner factors, and how long pregnancy has been attempted. The absence of regular menstruation does not mean that pregnancy is impossible, as unpredictable ovulation can still occur.
PCOS is not limited to menstruation or fertility. It may also be associated with insulin resistance, type 2 diabetes, abnormal cholesterol levels, high blood pressure, sleep problems, and emotional distress.
Not every patient develops these conditions, but follow-up should be based on individual symptoms and risk factors.
Sudden or rapidly worsening androgen-related symptoms may require additional testing for conditions other than PCOS.
