Ovarian Cysts / Endometrial Polyps

Ovarian cysts and endometrial polyps are common gynecologic findings that may be discovered during evaluation for pelvic pain, menstrual changes, abnormal bleeding, or fertility concerns. Although both may be found on pelvic ultrasound, they develop in different locations. Treatment depends on the type, size, symptoms, ultrasound findings, age, and pregnancy plans of each patient.

Overview

What Are Ovarian Cysts and Endometrial Polyps?

What Is an Ovarian Cyst?

An ovarian cyst is a fluid-filled or tissue-containing structure that develops in or on an ovary. Many ovarian cysts are related to the normal ovulation process and may disappear without treatment. Other cysts may persist, grow, cause symptoms, or require further evaluation.

Most ovarian cysts are benign, but persistent or complex ovarian masses may require follow-up or closer assessment.

What Is an Endometrial Polyp?

An endometrial polyp is a localized overgrowth of tissue from the lining of the uterus. Polyps may be single or multiple and may occur with or without symptoms.

They may cause irregular bleeding, spotting between periods, heavy menstrual bleeding, or difficulty becoming pregnant, although some are found incidentally during ultrasound or fertility evaluation.

Most endometrial polyps are benign. Management may include observation or polyp removal depending on symptoms, size, age, menopausal status, and pregnancy plans.

Cervical polyps develop on the cervix and may cause similar bleeding symptoms.

Symptoms & Causes

What Symptoms Can Occur?

A cyst or polyp may cause no symptoms, and symptom severity does not always reflect its size. Examination and ultrasound findings are therefore important.

Ovarian Cyst Symptoms

  • Changes in the amount, color, or consistency of vaginal discharge
  • Pelvic pressure, heaviness, bloating, or fullness
  • Pain during intercourse or around menstruation
  • Sudden pain if a cyst bleeds, ruptures, or twists
  • Changes in menstruation in some patients

Endometrial Polyp Symptoms

  • Bleeding or spotting between periods
  • Irregular, heavy, or prolonged menstruation
  • Bleeding after sexual intercourse
  • Bleeding after menopause
  • Difficulty becoming pregnant

Because ovarian cysts and endometrial polyps often cause no symptoms, regular gynecologic check-ups may help identify them before symptoms develop.

Why Do Ovarian Cysts and Endometrial Polyps Occur?

Causes of Ovarian Cysts

Ovarian cysts may develop as part of normal ovulation or from bleeding into a functional cyst. They may also be associated with endometriosis, pregnancy-related ovarian changes, hormonal or ovulation-related factors, or benign ovarian tissue growth. Less commonly, an ovarian mass may be related to borderline or malignant disease.

An ovarian cyst is not the same as polycystic ovary syndrome. PCOS is a hormonal and ovulatory condition and is not diagnosed simply because an ovarian cyst is present.

Causes of Endometrial Polyps

The exact cause of endometrial polyps is not always clear, but they are thought to be influenced by hormonal activity, particularly estrogen. They may be associated with increasing age, perimenopause or menopause, hormonal changes, obesity or metabolic factors, tamoxifen use, or other conditions affecting the endometrium.

An endometrial polyp may also occur when no clear risk factor is identified. Cervical polyps also have no single clearly established cause and may be associated with hormonal changes or chronic inflammation of the cervix.

Diagnose Method

How Are Ovarian Cysts and Endometrial Polyps Diagnosed?

Evaluation begins with a review of symptoms, menstrual pattern, pelvic pain, abnormal bleeding, medications, previous gynecologic conditions, and pregnancy plans.

Pelvic ultrasound is commonly used to assess both conditions. For an ovarian cyst, ultrasound evaluates its size, location, internal features, and whether it appears simple or complex. Follow-up ultrasound, blood tests, MRI, or referral may be recommended when the cyst persists or has concerning features.

A suspected endometrial polyp is usually evaluated with transvaginal ultrasound. Additional evaluation may be recommended depending on the ultrasound findings, symptoms, age, menopausal status, and individual risk factors.

Treatment

How Are Ovarian Cysts and Endometrial Polyps Treated?

Treatment depends on the type of cyst or polyp, its size, symptoms, ultrasound appearance, age, menopausal status, fertility plans, and individual risk factors.

How Are Ovarian Cysts Treated?

TypeHow treated
Observation and Follow-UpMany simple or functional ovarian cysts resolve naturally and can be monitored with repeat ultrasound. Pain medication may be used when symptoms are mild.
Hormonal TreatmentHormonal contraception may be considered in selected patients to suppress ovulation and help reduce the formation of new functional cysts. However, it does not necessarily make an existing cyst disappear.
Specialist ReferralAdditional evaluation or referral to a hospital that provides ovarian surgery may be recommended when a cyst persists or continues to grow, causes significant or recurrent pain, is large or complex, has solid components or other concerning ultrasound findings, causes complications such as torsion or significant bleeding, or occurs after menopause or raises concern for malignancy.

At LANTE OBGYN, ovarian cysts can be evaluated and monitored with ultrasound. When surgery or hospital-based treatment is needed, referral to an appropriate medical institution may be recommended.

How Are Endometrial Polyps Treated?

TypeHow treated
Observation and Follow-UpSmall polyps without symptoms may sometimes be monitored, particularly in younger patients with a low risk of endometrial disease.
Polyp RemovalRemoval may be recommended when a polyp causes abnormal bleeding, persists, is large, may affect fertility, occurs after menopause, or has concerning features.

At LANTE OBGYN, endometrial polyp removal may be performed after the polyp’s location and size have been assessed by ultrasound. The removed tissue may be sent for pathological examination when indicated. The procedure may be performed under sedation, and most patients can return home the same day.

When to Seek Care

When Should I Seek Prompt Medical Care?

Prompt evaluation is recommended if you experience:

  • Sudden or severe one-sided pelvic pain
  • Pelvic pain accompanied by nausea or vomiting
  • Dizziness, fainting, or marked weakness
  • Heavy or persistent vaginal bleeding
  • Fever with pelvic pain
  • Rapidly worsening abdominal swelling or pressure
  • Bleeding after menopause
  • Possible pregnancy with pelvic pain or bleeding

Sudden severe pain with nausea or vomiting may indicate ovarian torsion, rupture, or another condition requiring urgent treatment.

FAQ

Ovarian Cysts / Endometrial Polyps FAQ

Can an ovarian cyst be cancerous?
Most ovarian cysts are benign. However, complex features, solid components, growth, age, or menopausal status may require additional testing or specialist evaluation.
Is an ovarian cyst the same as PCOS?
No. An ovarian cyst is a specific structure in or on the ovary. PCOS is a hormonal and ovulatory condition and does not require a true ovarian cyst.
Can an ovarian cyst affect fertility?
Many ovarian cysts do not affect fertility. However, endometriomas, certain ovarian conditions, or surgery involving ovarian tissue may influence fertility depending on the individual situation.
Can an endometrial polyp affect pregnancy?
A polyp may interfere with implantation or be considered during fertility evaluation, although its effect depends on the size and location. Removal may be considered when pregnancy is desired.
Do endometrial polyps always need to be removed?
No. Small polyps without symptoms may sometimes be monitored. Removal may be recommended depending on bleeding, size, age, menopausal status, fertility plans, and other risk factors.
Can an endometrial polyp grow back after removal?
Yes. Polyps can recur after removal, so follow-up may be recommended if abnormal bleeding returns. Depending on the patient’s condition and underlying factors, hormonal medication may also be considered in selected patients to manage related symptoms or reduce the risk of recurrence.
