Vaginitis and cystitis are common conditions that can cause burning, abnormal discharge, urinary symptoms, or pelvic discomfort. Although some symptoms may overlap, vaginitis affects the vagina and vulvar area, while cystitis affects the bladder and lower urinary tract. Proper evaluation is important because treatment depends on the underlying cause rather than symptoms alone.
Vaginitis refers to inflammation or infection of the vagina. It may cause abnormal discharge, odor, itching, irritation, burning, or discomfort during urination or intercourse.
Common types include:

Because these conditions can cause similar symptoms, treatment should be based on examination and testing rather than appearance or symptoms alone.
Cystitis refers to inflammation of the bladder. The most common type is acute bacterial cystitis, which occurs when bacteria infect the bladder. Some patients experience recurrent cystitis, while persistent bladder pain and urinary symptoms without a bacterial infection may suggest a noninfectious condition such as interstitial cystitis or bladder pain syndrome.
Common symptoms include pain or burning during urination, frequent or urgent urination, passing only small amounts of urine, lower abdominal discomfort, and cloudy, bloody, or strong-smelling urine.
The pattern of symptoms may differ depending on the cause. For example, bacterial vaginosis may cause thin discharge and odor, while candidiasis more commonly causes marked itching, irritation, and thicker discharge. However, symptoms alone cannot reliably identify the cause.
Fever, chills, nausea, vomiting, or pain in the side or back may suggest that the infection has spread beyond the bladder and requires prompt medical evaluation.

Not every episode of vaginitis is sexually transmitted, and the cause cannot always be determined from symptoms alone.
Cystitis most commonly occurs when bacteria enter the urethra and reach the bladder.
Risk factors may include:
Recurrent cystitis may require evaluation for contributing hormonal, urinary, or structural factors.
The evaluation begins with a review of symptoms, timing, previous episodes, medication use, sexual history when relevant, and associated medical conditions.
Because vaginitis and cystitis can produce overlapping burning or urinary symptoms, both vaginal and urinary testing may be recommended.
Treatment depends on the cause and may include:
Using antifungal or antibiotic medication without identifying the cause may delay appropriate treatment, particularly when symptoms recur.
Bacterial cystitis is generally treated with an appropriate antibiotic based on symptoms, urine findings, and culture results when needed. Drinking adequate fluids may also support recovery.
Take antibiotics exactly as prescribed. Do not stop and restart them on your own or save leftover medication for later, as improper use may contribute to persistent infection and antibiotic resistance.
When vaginal symptoms persist or frequently return, the cause should be reassessed rather than repeatedly treating symptoms alone. Evaluation may consider recurrent infection, vaginal dryness, atrophic changes, irritation, hormonal factors, diabetes, medication use, or other vulvovaginal conditions.
Treatment may include repeated or extended courses of cause-specific medication, vaginal moisturizers, hormonal treatment when appropriate, and correction of contributing factors. In selected patients with chronic or recurrent symptoms accompanied by dryness, irritation, or atrophic changes, CO₂ vaginal laser treatment may also be considered as an adjunctive option after infection and other causes have been evaluated.
Recurrent cystitis may require further evaluation of urinary habits, sexual triggers, bladder emptying, menopausal changes, diabetes, urinary stones, or structural urinary tract conditions.
Treatment may include preventive medication, behavioral changes, menopausal treatment when appropriate, and additional urinary tract evaluation. In selected patients, intravesical treatment with hyaluronic acid and chondroitin sulfate, such as ialuril, may be considered to help restore the protective bladder lining and reduce recurrent or persistent symptoms. It does not replace antibiotics when an active bacterial infection is present.
