Nexplanon is a small, flexible hormonal contraceptive implant placed under the skin of the upper arm. It provides long-term pregnancy prevention without requiring a daily pill and may be removed at any time when pregnancy is desired or the patient wishes to change contraceptive methods. In Korea, the implant is used for up to three years.
Nexplanon is a single contraceptive implant containing etonogestrel, a type of progestin. The implant is a thin, flexible rod placed just beneath the skin of the upper arm.
Once inserted, it continuously releases a small amount of hormone into the body. It does not contain estrogen and may be considered for patients who prefer a long-acting method or who cannot use certain estrogen-containing contraceptives.
Nexplanon mainly prevents pregnancy by suppressing ovulation. It also thickens cervical mucus, making it more difficult for sperm to reach an egg, and changes the uterine lining.
Nexplanon prevents pregnancy before it occurs. It does not terminate an established pregnancy and does not protect against sexually transmitted infections.
Nexplanon is over 99% effective at preventing pregnancy. Fewer than 1 in 100 users become pregnant during the first year of use. Because the implant remains in place and does not require daily use, its effectiveness is not affected by missed pills.
In Korea, the implant may remain in place for up to three years and should be removed or replaced by the end of the third year. It may also be removed earlier at the patient’s request.
The timing of contraceptive protection depends on when the implant is inserted during the menstrual cycle and whether another hormonal contraceptive method was used previously. Condoms may be needed temporarily after insertion.

The most appropriate method depends on the patient’s health, bleeding pattern, medications, contraceptive goals, and future pregnancy plans.
Nexplanon insertion is a short in-clinic procedure performed under local anesthesia. The implant is placed just beneath the skin using a dedicated applicator, so a surgical skin incision is generally not required for insertion.
It is usually inserted into the non-dominant upper arm to reduce irritation or inconvenience during everyday activities. Patients who frequently perform strenuous upper-arm exercise, repetitive lifting, or activities involving pressure or trauma to the insertion area should discuss this with the doctor so that the placement site and aftercare can be considered individually.
After insertion, the doctor checks that the implant can be felt under the skin and applies a dressing to the area. Normal arm movement does not usually affect the implant, but pressure, rubbing, strenuous upper-arm exercise, and heavy lifting should be limited for a short period after the procedure.
Removal is performed under local anesthesia through a small skin incision near the end of the implant. The implant is then gently removed, and a dressing is applied.

If the implant cannot be easily felt or is located deeper than expected, imaging such as ultrasound may be needed to confirm its location before removal. Correct placement just beneath the skin is important for both effective use and straightforward removal.
A new implant may be inserted after removal when continued contraception is desired.
The most common side effect of Nexplanon is a change in menstrual bleeding. Periods may become irregular, lighter, heavier, longer, less frequent, or may stop completely. Spotting between periods may also occur, and the bleeding pattern can be difficult to predict.
Other possible side effects include headache, acne, breast tenderness, abdominal discomfort, mood changes, or changes in appetite or weight.
A change in bleeding pattern does not necessarily mean that the implant is ineffective. However, persistent heavy bleeding, severe pelvic pain, or other unusual symptoms should be evaluated.

Pregnancy should be excluded before insertion. Patients should also tell the doctor about all current medications, supplements, medical conditions, and previous reactions to hormonal contraception.
Rare complications may include infection, incorrect or deep placement, difficulty locating or removing the implant, nerve or blood vessel injury, or movement from the original insertion site. Correct insertion technique and confirmation that the implant can be felt beneath the skin are therefore important.
Mild soreness, bruising, swelling, or tenderness may occur around the insertion or removal site.
The clinic should be contacted if there is increasing pain, redness, swelling, discharge, numbness, or difficulty feeling the implant.
