Menopause is a natural transition caused by declining ovarian function and hormone production. It can affect menstrual patterns, temperature regulation, sleep, mood, vaginal and urinary health, sexual comfort, bone health, and overall quality of life. Evaluation is individualized according to age, symptoms, menstrual history, medical history, and examination findings.
Menopause is the permanent end of menstrual periods caused by a natural decline in ovarian function.
A woman is born with a limited number of ovarian follicles. With age, the number and activity of these follicles gradually decrease. As ovarian function and hormone production decline, ovulation becomes irregular and eventually stops, leading to menopause.
Menopause is a natural stage of life rather than a disease. However, its symptoms and longer-term health effects can be evaluated and treated when they affect daily comfort, sexual health, or overall well-being.
The menopausal transition commonly begins during the 40s, although its timing and duration vary. Natural menopause most often occurs between ages 45 and 55, with an average age of approximately 51.
Menopause before age 45 is considered early menopause. Loss of ovarian function before age 40 may indicate primary ovarian insufficiency and requires additional evaluation.
Earlier menopause may occur naturally or may be associated with ovarian surgery, certain medical treatments, genetic factors, autoimmune conditions, or other causes. In some patients, no clear cause is identified.
Menopausal symptoms vary widely. Some patients notice only menstrual changes, while others experience symptoms that affect sleep, work, sexual health, or daily life.
Possible changes include:
Reduced estrogen can affect the vulva, vagina, urethra, and bladder. These changes, known as genitourinary syndrome of menopause (GSM), may cause dryness, irritation, painful intercourse, and urinary symptoms and may persist or worsen without appropriate care.
Urinary Incontinence Evaluation
Not every symptom that develops in midlife is caused by menopause. However, if menstrual changes or symptoms such as hot flashes, sleep disturbance, vaginal dryness, or urinary discomfort begin around the expected menopausal age, a gynecologic evaluation can help identify the cause and allow appropriate care to begin early.
Menopause evaluation is based on age, menstrual changes, symptoms, medical history, and individual health risks. In many patients aged 45 or older with typical symptoms, menopause can be assessed clinically without routine hormone testing.
Additional testing may be recommended when menopause occurs earlier than expected, symptoms are unusual, or another medical condition needs to be excluded.
Treatment is individualized according to the most bothersome symptoms, medical history, examination findings, personal preferences, and potential benefits and risks.
